Welcome to Shepherd University's Sports Medicine website. Our Sports Medicine Staff currently consists of four BOC Certified Athletic Trainers who work under the guidance of our two team physicians through WVU Medicine. The staff is committed to ensuring quality healthcare for student-athletes while enabling safe and healthy participation in intercollegiate athletics.
Our Sports Medicine Department and its medical team will provide health care services to the student-athletes which include the following five domains:
· Prevention of athletic injuries
· Recognition, evaluation and immediate care of athletic injuries
· Rehabilitation and reconditioning of all injuries incurred by student-athletes
· Health care administration
· Professional development and responsibility
Our commitment is to provide this quality health care while educating the student-athlete on good health practices and injury maintenance. Maintaining continuous education, staying aware of new treatment options and a consistent commitment to improving our athletic training program, will assist us in offering a high standard of care.
Below on this website, you can find links to each form of our Annual Medical Packet. Each student-athlete is required to provide this necessary information before they are cleared to participate in any of our team's athletic activities. If you have any questions, please feel free to contact any member of our staff and we would be happy to assist you!

Concussion Information
Concussions and other traumatic brain injuries can be serious and a potentially life-threatening injury in sports. Research indicates that these injuries can also have serious consequences later in life if not managed properly. To combat these issues, Shepherd University has developed a concussion management protocol that will be used to help care for student-athletes that are suspected of sustaining a concussion.
The NCAA requires annual concussion testing for all student athletes. We use two different testing methods to gather baseline data for all our student-athletes. The first is an online test, ImPACT testing. The second is an in-person balance and coordination test.

Additional Sports Medicine Forms/Information
Mental Health is a key component of student-athletes wellness, and the athletic department can play a pivotal role in providing an environment that supports wellness. We require all our student-athletes to sign up to an application called
PROMotion. We are able to send weekly check-in surveys that can track their mental and emotional hurdles throughout the year. Based on those results we can aid in providing resources so that student-athletes can obtain referrals to mental health services.

All athletes that see a physician for any reason (illness, injury, etc..) MUST have a return to play form completed by the physician that treated them and return it to the sports medicine staff before they are considered cleared to participate in any activities relating to their sport.
• Return to Play Form
Any individual wishing to try out for/participate in a Shepherd University Varsity sponsored athletic event must be cleared by the sports medicine staff before participating. Please complete the attached waiver and return to the sports medicine staff for review before participating in any sport-related activity.
